new lib and apps directories

I added a mash/lib and mash/apps directory.  I have changed it so that
when you check out mash-code, you will get these files, but if you checked
out mash-code earlier, you will need to check them out manually.

From the top level directory (mash-code if you didn't change it), do:

cvs checkout lib
cvs checkout apps

Lib contains shared code such as the TclDp stuff.  Apps currently contains
just the amxd server code.
